We develop a plan of care that is appropriate, effective and meaningful for each client by assessing the client’s physical, financial, social and psychological needs. Once the evaluation is complete, the doctor will prescribe treatment and our care team members will create the care plan, which will be updated periodically when the client’s condition changes.

Our compassionate, trustworthy and certified Home Health Aids and Personal Care Attendants are available to assist with bathing, dressing, meal prep and eating, grooming, medication reminders, light housekeeping and transportation coordination. They are trained in all areas of personal care needs, are CPR certified and have met all requirements for employment, including background checks. They work hand in hand with the client’s nurses, physician, and therapists to ensure that each patient’s care plan is carried out according to the wishes of the patient, their family and the patient’s physician. Our personal home care team is here to give you peace of mind, knowing that your loved one is in good hands.
A smooth transition from the hospital to home, a rehabilitation facility, or a nursing home, can dramatically improve the outcome for patients as they move to the next level of care. Our team will help with the planning and implementation of the transfer, as well as the support necessary for the patient and family to ensure continuing recovery and improvement.
Companion Care provides emotional support and companionship for seniors who are generally healthy and want to remain independent in their home. It can also include a range of non-medical services, including basic cooking, exercises, memory games, shopping, a ride to recreation centers, to a hair salon or to visit a friend or a loved one. It also includes cleaning and support that help make a senior’s life more manageable.
Medications prescribed by your physician, when taken correctly, support the healing process. Knowledge and education about medications are key to maximizing their safety and effectiveness. Our home care team can oversee the administration of prescribed medications, educated the patient and family on proper dosages, frequency and side effecte, evaluate therapeutic responses to medications, assess for duplicate medications or drug interactions, and assist in developing a medication schedule to enhance compliance.
Physical therapy assists patients in regaining physical motion and strength. Many therapies can be provided in the home to improve recovery from illness, surgery, or injury. Our licensed physical therapists will evaluate the need for services and create a plan of care that allows clients to develop independence with daily activities. Conditions known to be improved with physical therapy services include, but are not limited to: falls, loss of balance, difficulty walking, joint pain, back pain, hip and knee replacement surgery, and any condition that causes a decline in function.
Darien Home Care provides highly skilled, compassionate nurses on a full, part-time or per visit basis, who work closely with the client, their physician, family members and care providers to maintain the client’s health, and prevent further illness and injury. Our nurses conduct patient and environment assessments, and coordinate care with all team members. They provide treatment and educate patients and other caregivers on the disease or injury process, treatment plan, medications, diet and nutrition, foot care, catheter and wound care.
